Care, in question

Questions before care

Keep the questions close

A short card for the visit

Bring this card to a visit with a prescriber you found. Leave room for their answers.

Ask, then keep the answer

Ask about

What the answer needs to cover

Who will prescribe?

What is your full name and role? Which state record can I check?

Which medicine?

What is the drug name, such as semaglutide? What is the product name, such as Wegovy? Is it a shot or tablet? Who makes it?

Why this choice?

What health goal are we treating? What in my history matters to that choice?

What is the full cost?

What is included? What is separate? What will I pay after the first offer ends?

What happens next?

How do I ask about a problem? When is the next review? What happens if this does not work for me?

These are questions to ask a prescriber, rather than answers to fill in from an advert.

Bring what helps the discussion

Make a note of the medicines and supplements you take. Add the names of past treatments and the reasons they ended.

Write down the health goal in your own words. It can be simple. Ask the prescriber to explain how the plan connects to that goal.

If a term is unclear, ask for a shorter word. If an answer is vague, ask for the exact name, the written terms or the next step.

Keep the answers together

Space for notes on a printed copy
Prescriber’s name and role
Exact medicine and form
Reason for this choice
Full cost after the offer
Follow-up plan and how to get help

Before agreeing

Ask for a copy of the plan. Put the medicine cost next to the care fee so they can be read together.

Keep any question that remains open. Ask when and how it will be answered. There is no need to turn an unclear answer into a yes.
